Our partnership offers a strategic link for the planning and provision of health services to member agencies within the cities of Kingston and Bayside in Melbourne's South East.

 

Primary Care Partnerships are an initiative of the State Government to facilitate the development of stronger partnerships, support service coordination and integrated chronic disease management and promote the integration of health promotion across agencies .

The Victorian Public Health and Wellbeing Plan 2011–2015 has been prepared, as required by section 49 of the Public Health and Wellbeing Act 2008.

 

The plan aims to improve the health and wellbeing of all Victorians by engaging communities in prevention, and by strengthening systems for health protection, health promotion and preventive healthcare across all sectors and all levels of government.
