There is a high level of trust between partners and the capacity to enhance the members for mutual benefit and a common purpose. Mental Health and Community Connectedness Community ConnectednessThis working group started as the Depression and Older Persons Working Group under the priority area of mental health and wellbeing in the first Community Health plan. Due to the change of focus and structure the group has changed its name and is now more inclusive of population groups. The aim of this working group is to look at social inclusion and social isolation. The group has produced a logo for use by agencies. Guidelines for use of the logo can be found here. Future Focus for Youth Future Focus for Youth Working group formed in late 2006. This group came together due to strong interest and commitment to “at risk” youth, particularly youth that were homeless or at risk of being homeless. Since forming the group has worked hard developing a partnership. The first activity was to do a Capacity Inventory of agencies. The next step was to do a focus group with young people. This has provided the group with a lot of information which they now have to work through and establish their direction. Young people have joined this group as community representatives. These are the first community representatives in any for the working groups except. These young people need continued support in their role and this is to be provided by their agency. Talking Realities ….young parenting is an innovative school based, peer education program developed in South Australia. The program trains young parents as peer educators to share their stories and experiences with other young people and in so doing offers a realistic awareness of the potential short and long term consequences of pregnancy and parenting at a young age. Kingston Bayside PCP purchased the program in 2004 with School Focused Youth Services funding, following which 3 year funding until June 2009 was secured through Dept Family, Community Services and Indigenous Affairs. Active for Life Physical activity has been a priority in its own right since 2004. Prior to this it was connected to Mental Health and Well Being and this is when Southland Striders was first established. Physical Activity remains a priority for the 2006-2009 Community Health Plan.
