Continuous Improvement Framework
The Service Coordination Continuous Improvement Framework is a tool which can be used to assist agencies in Victoria implementing Service Coordination.
   
Elder Abuse Prevention Strategy (EAPS)

The Elder Abuse Prevention Strategy (EAPS) has been developed to protect and safeguard the rights of older Victorians.  The fundamental principle underpinning the strategy is that every Victorian has the right to live safely and to be treated with dignity and respect

 

A key component of the EAPS Strategy is the Primary Care Partnerships Community Awareness Grants Program, which aims to empower senior Victorians and ensure the development of a collaborative response across government departments, service and support sectors, older people and their families to prevent and address elder abuse.

 

An element of the Community Awareness Grants Program is to support the development of agency policies and procedures to respond and act on suspicion of elder abuse and to develop and review interagency protocols that enable cooperation in responding to elder abuse.

Good Practice Guide

The Good Practice Guide sets out the agreed good practice of Service Coordination across Victoria. The Practice Guide has been designed for practitioners involved in the delivery of Service Coordination. It is based on the Victorian Service Coordination Practice Manual and complements the Continuous Improvement Framework.

 

HACC/ACAS and Packaged Care Memorandum of Understanding

The Southern Metropolitan Region (SMR), HACC, ACAS, Packaged Care Management Group, is pleased to announce that the revised HACC, ACAS, Packaged Care Memorandum of Understanding (MOU) is now available.

 

The MOU and Practice Protocols may be used by organisations, including Local Interface Networks (LINs), to address issues in relation to referral, assessment and ongoing service delivery for clients. Using this document as a guide should ensure clearer communication pathways and better outcomes for clients.

 

All relevant organisations working together in the Southern Metropolitan Region of Melbourne are invited to sign the MOU and follow the protocols.

 

Victorian Service Coordination Practice Manual

The Victorian Service Coordination Practice Manual defines the common practices, processes, protocols and systems which support Service Coordination across Victoria. It replaces all local and regional Service Coordination Practices, Processes, Protocols and Systems (PPPS) manuals (16 different manuals were in existence in May 2006)

Integrated Chronic Disease Management (ICDM)

Divisions of General Practice - Self-Management Mapping

An identified ICDM priority for PCPs in working with and supporting agencies is support to enhance the provision and coordination of self-management approaches across their catchments. All PCPs have now collected information that maps the self-management approaches used within their local catchments. A statewide summary of this activity is available to be downloaded.

 

   
ICDM Workforce Capacity Building Strategy
The Primary Health Branch has developed a draft ICDM Workforce Capacity Building Strategy to respond to the need to support the systems changes required of the Early Intervention in Chronic Disease (EIiCD) and the ICDM initiatives.
