Service coordination is underpinned by a common set of forms and guidelines for the human services and a common form for use by general practitioners. Here, we provide links to these common forms.

Service Coordination Tool Templates

The Service Coordination Tool Templates (SCTT) is a suite of templates developed to facilitate and support service coordination. Using the SCTT can improve communication between service providers, the recording of information generated by screening and assessment processes, information sharing, and the quality of referrals and feedback between service providers. This can assist service providers to share relevant information to support better outcomes for consumers.

SCTT 2009 User Guide

The guide is a practical resource to assist in the use of the templates. It provides:

an introduction and overview of the templates
points for service providers to note in completing them
the code sets needed to complete the SCTT; and
an overview of the general practitioner’s referral tool, the Victorian Statewide Referral Form

Carer's Profile

Commonwealth Carer Respite Services has asked that agencies in the SMR use a Carer Profile for referral into the Commonwealth Carer Respite Services. This Profile is currently being used in regions across Victoria and provides an interim solution for Carer Respite Services awaiting the development of state wide Carer Profile as part of SCTT. Victorian Statewide Referral Form

The Victorian Statewide Referral Form (VSRF) is a straightforward, easy to use, electronic referral form for GP referrals to other services. The referral form provides a standardised mechanism for referral from general practice to health providers including hospital specialist clinics (outpatients).


Integrated Chronic Disease Management

Early Intervention in Chronic Disease in Community Health (EIiCD)

EIiCD focuses upon community based early intervention services for people with chronic diseases. The initiative invests in both Community Health Services and Primary Care Partnerships.

HARP - Chronic Disease Management

The Victorian Government established the Hospital Admission Risk program (HARP) in 2001-02 to develop preventive models of care involving hospitals and community agencies which focused on people with chronic and complex conditions and gave priority to high volume and/or frequent users of the acute public hospital system.

HARP CDM focuses on continuing to improve the management of people with defined chronic diseases and complex needs who frequently use hospitals or who are at risk of hospitalisation. HARP CDM involves embedding the models of care that have emerged through HARP since 2001 into the Victorian service system.
