HACC, ACAS and Packaged Care Memorandum of Understanding

The Southern Metropolitan Region (SMR), HACC, ACAS, Packaged Care Management Group, is pleased to announce that the revised HACC, ACAS, Packaged Care Memorandum of Understanding (MOU) is now available.

 

The MOU and Practice Protocols may be used by organisations, including Local Interface Networks (LINs), to address issues in relation to referral, assessment and ongoing service delivery for clients. Using this document as a guide should ensure clearer communication pathways and better outcomes for clients.

 

All relevant organisations working together in the Southern Metropolitan Region of Melbourne are invited to sign the MOU and follow the protocols.

 

To mark this occasion and acknowledge the extensive work done by the Management Group, there will be a launch of the revised MOU in early December.


 


Primary Care Partnerships: Achievements 2000 to 2010

The Department of Health has recently released a report summarising the achievements of PCPs Achievements 2000 to 2010. The report demonstrates the  contribution of PCPs to developing the  primary health care system in Victoria. Evidence used to describe the achievements of PCPs has been drawn from a range of sources including the PCP annual reporting requirements.

Highlights of the report include: the engagement of 1200 health and human services organisations; continuously improving service coordination practice; improved quality and effectiveness of health promotion; and implementing an evidenced-based approach in responding to chronic disease.

South East and Bayside Diabetes Alliance – SEBDA

A body of work undertaken in 2008 by Alfred Health investigating the impact of diabetes on their services and across the service system lead to the formation of the South East Bayside Diabetes Alliance (SEBDA). The alliance comprises twelve key agencies responsible for diabetes services across the Inner South East and Bayside catchments who in 2009 agreed to work in partnership to improve care continuums and collaboration across health care settings for the management of diabetes.  The agencies involved include primary care partnerships, local divisions of general practice, hospitals, community health services and diabetes peak bodies.
